Ontario Ministry of Health and Long-Term Care

Health Card
Price: $No Fee

To change your name on your health card to reflect your birth name or a previous married name, you must visit a ministry office.

 

 

 

Marriage Licence Applications

Social Insurance Number
Price: $No Fee

To apply for an amendment Social Insurance Number record or card, you must complete an application form. You can obtain an application from your local office or download one.

 

 

Ministry Of Transportation
Drivers Licence Name Change  

You must notify the ministry within six days of changing your name. When a person is married, the married name is considered an assumed name. You may change your married name with the ministry when you receive your government issued marriage certificate. The six-day rule does not apply in this case.
